This payment processing is for change request of existing licensed facilities only.  If you are a new applicant, please go back to Health Care Providers (michigan.gov).  Payments may be made with a valid Visa, MasterCard, Discover, or American Express with a 2.25% processing fee or by electronic check (echeck) for $0.15.  Echeck transactions are electronic payments processed directly from your checking or savings account.     To begin the payment process, enter your information and click the “Next” button in the box below.
